1)Here is a list of items that you will need to bring with you to enroll your child in Morrison Community Unit District #6.

Consent for Release of Records

Proof of Residence - Please bring two forms of proof. Examples - Utility Bill, Rental Agreement, Mortage Paperwork
Proof of Gaurdianship if you are not the parent
Vacination Records
Transcript or Report Card
Birth Certificate
Health Examination
Eye Examination
Dental Examination
If your child needs Medication at school
Authorization for Administration of Medication
School Medication Authorization

Please complete and bring these items to the High School, 643 Genesee Ave. If your child is attending the Junior High or Elementary Schools, these schools will reopen after August 3rd.

Junior High (6th-8th grade) 300 Academic Drive
Southside Elementary (3rd-5th grade) 100 Academic Drive
Northside Elementary (PreK-2nd grade) 520 N. Genesee Street
